Please complete the appropriate form if the item is initiated on your To Do list in Opus. A link to the Emory Financial Aid page, where you can download forms is here.

IDOC Instructions:
1. Go to the IDOC website: https://idoc.collegeboard.com
2. Click on the "IDOC" button.
3. Log in by entering two of the following: IDOC ID, SSN, and date of birth.
4. Click on the "IDOC Requirements" link and print this information.
5. Click on the "IDOC Cover Sheet" link and print on one side of a single sheet of paper. This cover sheet must accompany all documents that you send to IDOC.
6. Mail your IDOC packet to: The College Board IDOC, P.O. Box 4017, Mt. Vernon, IL 62864

Documents to mail to IDOC:
1. Copies of Custodial Parents' signed 2012 federal tax forms, including W2's, 1099's and all related schedules, along with any corporate and partnership returns that were submitted to the IRS.
2. Copies of Students' signed 2012 federal tax forms, including W2's, 1099's and all related schedules, along with any corporate and partnership returns that were submitted to the IRS.
