Confidentiality
with SHS:
No one except the healthcare providers have access to your health information. Information and records regarding your treatment are privileged and will not be disclosed without your written permission. If you are 18 or older, even your parents will need permission from you to inquire or talk with healthcare providers about your care.
All applicants who accept the college's offer of admission must submit a completed Entrance Medical Record to the Student Health Service prior to registration. Mental health records are maintained separately and are not included in the student's general medical file. Information in the medical record is confidential and will not be released to anyone without written consent from the student.
